An inquiry by management consultants Price Waterhouse has concluded that the main reason for an error which lost Middlesbrough BC£557,358 housing subsidy was individual mistakes and not systematic control weaknesses.

Changes created by local government reorganisation meant the risk of mistakes was increased, although some control weaknesses were uncovered.

Two officials at the council have been disciplined as a result of the independent inquiry, The Northern Echo reported (Jan 12, p3).
