The Scottish Executive is to overhaul children's services after a report revealed vulnerable children are being put at risk.

A ministerial task force has been set up after a report from local government, health and voluntary sector staff identified failings in the existing system.

The report, For Scotland's children, said services and assessment of need are often poorly coordinated.

It said: 'Some children are effectively invisible to services. There continue to be problems in sharing information.'

Education minister Jack McConnell (Lab) said a plan has been produced.

He added: 'We need to find better ways of working effectively together.'
